Contractor Registration Requirements

1)      Copy of state license

2)      Copy of occupational license

3)      Copy of Lake County competency card

v     Required If Registered and Not State Certified

4)      Insurance certificates with the Town of Lady Lake listed as certified holder for the following:

v     General Liability

v     Workman’s Compensation or Letter of Exemption

5)      Company’s physical address and phone number

v     No P.O. Boxes

6)      Power of Attorney for anyone that is going to pick up permits that is not the license holder

v     Power of Attorney MUST be Notarized

This information may be faxed to the Building Department at (352) 751-1514.  Insurance certificates must come directly from the insurance company.
